“Pharmacovigilance is the science of collecting, monitoring, resear ching, assessing and evaluating information from healthcare providers and pa tients on the adverse effects of medicines, biological products, herbals and traditional medicines, with the view to; Identifying new information about hazards, and Preventing harm to patients.” At the end of this lear ning unit, the participants will be able to define pharmacovigilance and its importance, describe the reporting system for pharmacovigilance and succefu lly individually report pharmacovigilance information using relevant tools. The pharmacovigilance reporting tools include the adverse drug reaction (yel low form), poor quality medication (pink form) and the alert card (white fro m). The scope of  pharmacovigilance involve: Product development, S ubstandard and counterfeit medicines, Medication error reporting, Assessment of medicine-related mortality, Abuse and misuse of medicines reports, Effic acy monitoring, Off-label use of medicines, Case reports of acute and chroni c poisoning and Adverse interactions of medicines with chemicals, other medi cines, and food reports. Burden of ADRS in Kenya is about15-25%; specifi cally among patients on HAART. This has led to change of regimen to alternat ive ARV drug as a result of toxicity over 12 month period in the year ending 2015. Consequently, in report ADRs and poor quality medicines we do not nee d to be certain to report BUT just be suspicious!!!
