Description

approach to treatment of organophosphate poisoning

Objectives

Explain the mechanism of toxicity of organophosphates and their effects on the nervous system. Recognize the clinical manifestations of organophosphate poisoning, including muscarinic, nicotinic, and central nervous system features. Outline the initial assessment and resuscitation of patients with organophosphate poisoning using the ABCDE approach. Describe the principles of decontamination and appropriate personal protective measures to prevent secondary exposure among healthcare workers. Discuss the indications, dosing, and administration of atropine, including clinical endpoints for atropinization and the management of recurrent cholinergic toxicity. Explain the role, indications, and appropriate administration of oximes, particularly pralidoxime, in the management of organophosphate poisoning. Identify indications for airway protection, supplemental oxygen, and mechanical ventilation in patients with respiratory compromise. Discuss the management of seizures, agitation, and other complications associated with severe organophosphate poisoning. Describe the appropriate monitoring, supportive care, and criteria for escalation of care in critically ill patients.
