Acute disseminated encephalomyelitis (ADEM) is an acquired inflammatory demyelinating syndrome that predominately affects the white matter of the brain and spinal cord in a multifocal distribution. Clinically, ADEM manifests as subacute-onset of encephalopathy associated with polyfocal neurologic signs and symptoms. ADEM may mimic other acute demyelinating syndromes (ADS) of childhood, including multiple sclerosis (MS); however, ADEM is typically distinguishable from alternative diagnoses on the basis of clinical features and findings on neuroimaging and laboratory investigations. With the discovery of antibodies targeting myelin oligodendrocyte glycoprotein (MOG), ADEM is one of the most common phenotypes of MOG antibody disease (MOGAD) in children
