Description

This mentorship-based CME aims to enhance participants’ understanding and practical application of haemovigilance — the systematic monitoring, reporting, and prevention of adverse transfusion events across the entire blood transfusion chain. The session emphasizes the role of healthcare workers in detecting, documenting, and analyzing transfusion reactions and near-miss events to improve patient safety and transfusion outcomes. Through interactive mentorship, participants will gain skills in establishing and operating haemovigilance systems at both facility and national levels, conducting root-cause analysis, implementing corrective and preventive actions, and using haemovigilance data for quality improvement and policy development in blood services.

Objectives

By the end of this mentorship activity, participants should be able to: Conceptual Understanding Define haemovigilance and describe its scope within the blood transfusion chain — from donor recruitment to recipient follow-up. Explain the importance of haemovigilance in improving transfusion safety, quality assurance, and regulatory compliance. System Establishment and Reporting Outline the structure and core components of a functional haemovigilance system at institutional and national levels. Identify and classify adverse transfusion reactions and near-miss events according to standardized nomenclature (e.g., ISBT/IHN). Demonstrate proper completion and submission of haemovigilance reporting forms. Investigation and Data Analysis Conduct systematic investigation of reported transfusion incidents, including root-cause analysis and risk assessment. Utilize haemovigilance data for trend analysis, feedback, and continuous quality improvement (CQI). Quality and Patient Safety Integration Implement corrective and preventive actions (CAPA) to reduce recurrence of adverse events. Integrate haemovigilance with hospital transfusion committees, laboratory quality management systems (QMS), and clinical governance frameworks. Mentorship and Sustainability Strengthen capacity of transfusion and clinical staff through peer mentorship and continuous professional development. Advocate for institutional support and national coordination of haemovigilance activities for sustainability.
