Description

Introduction Defined as respiratory distress in newborn infants delivered through meconium- stained amniotic fluid, whose symptoms cannot be otherwise explained. Can present with varying degrees of severity. MSAF is seen in about 8-20% of births MAS occurs in 2-10% of newborns delivered through MSAF It is a disease of term and post-terms neonates Mechanism of passage is unknown, depend on increased intestinal peristalsis, relaxation of the anal sphincter, and a gestational age greater than 34 to 35weeks. Increased peristalsis may be caused by increased concentrations of motilin or triggered by infection, hypoxia, or vagal stimulation produced by sporadic or repetitive cord compression, any of which also may result in sphincter dilation Post-term fetus with mature GI tracts with none to minimal distress may also pass meconium

Objectives

Management- Prenatal Prenatal management- key management lies in the prevention during the prenatal period Identification of high-risk pregnancies and close monitoring. For pregnancies that are continue past due date, early induction before 41 weeks (Some guidelines now advocate for delivery at 39 completed weeks. Post-datism is associated with higher morbidity/mortality in comparison to term babies Corrective measures (Expedited delivery) should be taken promptly incase of fetal distress. Management- Post natal Well babies born through meconium do not need the much intervention, just close monitoring of the baby and the vitals For ill babies- Transfer to NICU/ NBU Monitor closely Respiratory support Management of patients with risk of neonatal sepsis and full work up Feeds and fluids management Minimal handling to avoid agitating the neonate Maintain and optimal thermal environment
