Objectives

Critically review cases with adverse outcomes to identify common patterns, contributing factors, and areas for improvement in internal medicine practice. Improve diagnostic processes by learning from cases where misdiagnosis or delayed diagnosis contributed to poor outcomes. Incorporate lessons learned from M&M reviews to update or develop clinical guidelines and protocols. Examine cases with adverse outcomes in the ICU to identify system failures, procedural errors, and areas needing improvement. Review surgical cases with complications or poor outcomes to identify contributing factors and areas for improvement.
